Patient Eligibility & Requirements
Testing is available only for patients who are eligible and either symptomatic, have known exposure to a medical condition, or have been evaluated by a doctor and determined to have a valid medical necessity for testing.
Patients must show a valid government-issued photo ID at the time of testing.
Patients must have an active insurance plan. A valid insurance card must be presented.
In order to proceed with the test, patients must fill out the consent form and intake form and be evaluated by a health care professional.
